At the time of appointment, a guardian will receive from the court a notice of duties. The notice of duties details the annual reporting requirements of the guardian based on the Order Regarding Appointment of Guardian of Incapacitated Individual signed by the Probate Judge. Please review the notice of duties or contact Probate Court if you are unsure which documents are needed for reporting. Reports are due annually on the date specified in the notice of duties and on the Letter of Guardianship.
