Passport Information

PASSPORT PROCESSING HOURS(By appointment)
Monday - Friday
8:30 A.M. - 11 A.M. and 1:30 P.M. - 4 P.M.
Note: The availability of a passport agent cannot be guaranteed after 4 p.m
Additionally, there may be occasions when there is no passport agent available. Please call in advance to be assured.
Clinton County Register of Deeds Office is a certified United States Passport Acceptance Agency. Please note that official U.S. Passport information is available on the U.S. Department of State website.
GENERAL INFORMATION
Adults 18 Years of Age & Older
- Passport Application - May be obtained from the County Register of Deeds Office or downloaded from the U.S. Department of State at the following link: Passport Information
Applications must be printed
1 sided is acceptable
It is important to use black ink when hand filling applications
- Proof of U.S. citizenship in the form of a certified birth certificate, previous passport or naturalization certificate AND a photo copy. If there is printing on both sides of the citizenship evidence, then copies must be made of the front and back of the document but cannot be double-sided.
- Proof of Identity (Photo ID) - Includes a valid driver's license, a government, or military ID.
- Passport quality photo.
- Appropriate fees.
- Fill out a Previous Marriage Information Form (PDF), if applicable. (One form for each prior marriage).
- If a previous passport has been lost or stolen, an additional form will need to be completed pertaining to that passport. This form can be obtained at the County Register of Deeds Office or downloaded from Passport Information. Note: By clicking on this link you will be directed to the Department of State's website.
Minor Child of Age of 16 or 17
- Passport Application - May be obtained from the County Register of Deeds Office or downloaded from the U.S. Department of State at the following link: Passport Information. Note: By clicking on this link you will be directed to the Department of State's website.
Applications must be printed

- Minor child age 16 or 17 must appear in person and sign his/her own passport application.
- Proof of U.S. citizenship in the form of a certified birth certificate, previous passport, or naturalization certificate AND a photo copy. If there is printing on both sides of the citizenship evidence, then copies must be made of the front and back of the document but cannot be double-sided..
- One parent must be present with the child at the time of application and sign the application.
- If applying parent is the child's mother and mother's last name is different from the child's due to a marriage, a certified copy of the marriage license is required.
- Proof of identity (Parent's photo ID and applicant's, if applicable).
- Passport quality photo.
- Appropriate fees.
- If a previous passport has been lost or stolen, an additional form will need to be completed pertaining to that passport. This form can be obtained at the County Register of Deeds Office or downloaded from Passport Information. Note: By clicking on this link you will be directed to the Department of State's website.
Minor Child 15 Years of Age & Younger
- Passport Application - May be obtained from the County Register of Deeds Office or downloaded from the U.S. Department of State at the following link: Passport Information.

When submitting a previous passport, a certified birth certificate must be included showing parents' names on the record.
- Both parents must be present with the child at the time of application.
- If mother's last name is different from the child's due to a marriage, a certified copy of the marriage license is required.
- Proof of Identity (Both parents' photo ID).
- Passport quality photo.
- Appropriate fees.
- If a previous passport has been lost or stolen, an additional form will need to be completed pertaining to that passport. Additional Information
- All applications require both parents names, dates and places of birth.
- Provide proper documentation of any legal names used, i.e. certified copies of court orders for name change or adoption. (A name change as a result of a marriage or divorce does not apply.)
Fees
Applicants 16 Years of Age or Older
- $130 check or money order payable to the "U.S. Department of State."
- $35 cash, check or money order payable to "Clinton County Register of Deeds."
Applicants 15 Years of Age or Younger
- $100 check or money order payable to the "U.S. Department of State."
- $35 cash, check or money order payable to "Clinton County Register of Deeds."
Photos
May be taken at the Register of Deeds Office for $10 from which 1 passport quality photo will be given to you. The U.S. Department of State requests that you NOT wear a white top in passport photos.
